心衰竭 (Heart Failure, HF) 是心臟功能越來越差，漸漸走向失能的過程。心衰竭儼如心臟的癌症，經診斷後五年存活率約只有50%，再住院率高且活動功能嚴重受限，病人生活品質極差。

目前臨床上針對HF有兩種分級，一種是ACC/AHA心衰竭分期標準，我們會說 Stage A/B/C/D

ACC&AHA心衰竭分級

Stage A : There is the risk of developing heart failure (HF), but no actual structural disease is present
Stage B : Structural disease exists but it is not causing any HF symptoms.
Stage C : Structural disease is present and is causing symptomes of HF.
Stage D : End-stage HF is present.

其中，Stage A 與 Stage B 可以說沒有HF的事實，但是具有HF的風險，因此在此階段就要預防HF的產生。

另一種是針對心衰竭病人的功能分類，NYHA對於心衰竭的功能分級標準，我們會說 Class I / II / III / IV

解析

(A) 因為心肌肥厚，分母變多了，所以 perfusion 減少。
(B) 正確。病理切片看 MI 可以發現淋巴球、巨噬細胞，所以缺血確實會引起發炎反應。
(C) Angiotensin II 增加不是減少。
(D) LV wall 增厚。

(A) NYHA function IV
(B) low plasma norepinephrine

解析

交感神經是心臟衰竭的代償機制之一，交感神經的啟動程度可以做為預後的判斷分子之一，可以用 plasma norepinephrine (PNE)作為判斷的因子。

(B) 右心衰竭要給水不要給利尿劑。利尿劑增加 K+ & H+ 排出，H+排出造成代謝性鹼中毒。
(C) Phlebotomy (放血治療) 適應症：Hct > 65%。（Normal Hct: 男=40～50％，女=35～45％。）
(D) Digoxin適應症: HF、AF。但安全劑量範圍窄，容易導致Digoxin中毒。
