呼吸音 (Breathing sound)，屬於聽診 (Auscultation) 的一種，是胸腔理學檢查的一環。

考題觀摩

1. 假如聽到兩側肺葉有 bibasilar end-inspiratory dry crackles，可以下什麼診斷？
(A) 間質性肺部疾病 Interstitial lung disease→有特殊的魔鬼沾撕裂音end-inspiratory dry crackles
(B) 非典型肺炎 Atypical pneumonia →肺葉 consolidation，應該是單側crackles
(C) 肺水腫 Pulmonary edema →因為水水的，應會是雙側crackles
(D) 急性呼吸窘迫 ARDS →是Type I 肺泡細胞壞死，不會有水，聽起來乾乾的

解析：選(A)。

2. 一位2歲兒童因為喉嚨沙啞、狗吠狀咳嗽而來醫院就診，檢查生命徵象體溫36.7°C 、心跳104bpm、呼吸速率32/min。醫師安排X-ray攝影如下，請問預期會聽到何種呼吸音？

解：哮喘Croup是3m/o-5y/o常見的疾病，診斷靠臨床診斷：下列符合3項 (1)Inspiratory stridor (2)Hoarseness (3)Barking cough。哮喘是由parainfluenza virus type I-III感染造成的。X-ray會看到特殊的Steeple sign(尖塔徵象)。如果沒有呼吸窘迫，給予Nebulized epinephrine治療即可；若出現呼吸窘迫給予Dexamethasone IM or Nebulized budesonide。
